Privacy Policy

We strive to anticipate changes in market drivers and to work with our clients and employees to adapt to those changes

This privacy policy (the “Policy”) describes the information (including patients’ identifiable health information) that Sentinel Healthcare Systems, Inc., (“Sentinel”) collects from and about our customers when they use the Healthware site. Sentinel collects the following types of information (“Personal Information”) from customers during their use of the Healthware site:

Identity (user name and password)
Location (IP Address)

The Policy applies to the information you collect online but does not apply to information collected in different forums, including information collected offline.
Please be sure to read this privacy policy before using or submitting information to the Healthware site. The Policy may change from time to time, so please check back periodically. By accessing and using our website, and by providing us with your information, you agree to be bound by this Policy.

The following statements describe how Sentinel uses and protects that information:

1. Sentinel combines physical, technical and administrative safeguards to help prevent the loss, misuse or alteration of Personal Information (as defined in paragraph 2, below) under our control. Except for circumstances where Sentinel is legally required to provide access to agents of governmental health oversight, public health, law enforcement, courts and judicial agencies, or other persons, only authorized Sentinel employees, agents contractors or affiliates are permitted to access Personal Information. Sentinel requires all of its employees, agents, contractors and affiliates who access personal Information to agree in writing to abide by this Policy, and subjects those who violate the Policy to disciplinary action, up to and including termination. In addition to its written policies and procedures, Sentinel employs physical and technical security measures to safeguard online transactions; Personal Information is always encrypted when sent via Internet channels, and resides on systems stored in facilities with controlled physical access.

2. We protect the integrity of data collected through Healthware and stored on Sentinel systems by implementing security measures on operating systems, virus protection, using firewalls and intrusion detection, and by routinely monitoring electronic and physical access to our systems We also have a plan in place to mitigate catastrophic risks and malicious attacks.

3. The only information Sentinel obtains about an individual user to its Web Site is information supplied voluntarily by that user.

4. Sentinel will not disclose Personal Information to anyone unless (a) the user indicates that Sentinel may do so; (b) as part of the services requested through the Healthware site; (c) as part of a sale merger or other change of ownership of any part of Sentinel’s business that includes the Healthware site; or (d) the disclosure is required by law or legal process. Sentinel may share statistical information about its users as a group, such as usage habits, demographics and similar information that is stripped of components that Sentinel believes could permit direct identification or contact users (\"Aggregate Information\"), with any affiliate of Sentinel.

5. Personal Information about an individual patient or resident of a customer relating to Healthware transactions, reports and other summaries derived from Healthware transactions, or within administrative information related to Healthware transactions, that can be used to identify or contact that individual will be protected by Sentinel as \"Individually Identifiable Health Information\" in accordance with the requirements and definitions of the administrative simplification provisions of the Health Insurance Portability and Accountability Act (“HIPAA”) of 1996 (42 U.S.C. §1320d-2(d)) and any implementing regulations. Sentinel intends that its use and disclosure of such information will meet HIPAA standards and applicable state and federal laws relating to transmission, storage, and access to medical records and other health care data.

6. Sentinel may place a text file called a \"cookie\" in the browser files of your computer. Cookies are pieces of information that a web site transfers to an individual\'s hard disk for record keeping purposes. The Healthware Site uses cookies during your online session to confirm that access to our site is authorized and to control certain parameters of your communication session with our Site. Sentinel will not use cookies for advertising purposes.

7. An IP address is a number assigned to your computer for accessing the Internet. All computer identification on the Internet is conducted with IP addresses, which allow computers and servers to recognize and communicate with each other. Sentinel collects IP addresses from firewall logs in order to conduct system administration, report aggregate information, to conduct retroactive intrusion detection analysis, and to conduct site analysis. If a user requests pages from the Healthware site, Sentinel’s servers enter the user\'s IP address into a log. To maintain user privacy, Sentinel does not associate IP addresses with records containing Personal Information.

8. Except as set forth herein, Sentinel will not disclose to unaffiliated companies any Personal Information about our users and customers. Any Personal Information provided to entities affiliated with Sentinel will be treated in accordance with the terms of this Policy.

9. Sentinel requires users to register with Sentinel in order to access specific portions of the Healthware site. Registration includes the collection of contact information such as user name and e-mail address. Registration information will be stored on secure servers behind a secure firewall. Sentinel’s policies, procedures, and physical and technical security safeguards apply to registration information in the same manner as other Personal Information.

10. Sentinel may change this Policy at any time by posting revisions to our Healthware site. Your use of the Site constitutes acceptance of the provisions of this Policy and your continued usage after such changes are posted constitutes acceptance of each revised Policy. If you do not agree to the terms of this Policy or any revised policy, then please exit the Site immediately.

If at any time you believe that Sentinel has not adhered to these principles, please notify us at support@sentinelhealthware.com or (800)-795-6571 and Sentinel will use good faith efforts to promptly identify and correct any deviations from the Standards described in this Policy.
